Job summary The ERAS Plus Nurse contributes to the provision of specialist nursing for Colorectal and Urology patients, practicing within the multi-disciplinary team.

Providing psychological care to the patient undergoing major abdominal surgery.
Provides specialist advice and education to patients, families, carers and staff on Enhanced Recovery +, in both hospital and the community.
Assesses patients, plans, implements care, provides specialist advice; maintains associated healthcare records.
Co-ordinates other healthcare professionals involved in the care of Colorectal/Urology Enhanced Recovery Patients, including surgeons, anaesthetists, pre-op assessment team, acute pain service, stoma care service, physiotherapists, dieticians and ward nursing staff (does not line manage these staff).
Provides specialist education & training to other staff and students Undertakes research and leads clinical audits in own specialist area.

Analyse relevant national policy, advice on impact for service and lead the implementation of changes and evaluation of processes in partnership with the ERAS+ leads and MDT.
Provide specialist clinical advice to all grades of medical, nursing staff and multi-disciplinary team members within surgery.
Formulate, implement and review new policies as required to improve standards in clinical practice in all aspects of ERAS+.
Plan and manage your own workload and the workload of the service in a flexible manner ensuring that service provision meets the needs of the patients.
Ensure appropriate verbal and written information is available to all patients and relatives in relation to ERAS+.
Assist in the development of business plans for developments in ERAS+ with the appropriate divisional business managers.
Coordinate and chair Trust wide ERAS+ meetings In collaboration with the Stepping Hill audit team ensure regular and robust monitoring and auditing of the quality, benefits and productivity achieved within ERAS+ Alongside the Colorectal CNS monitor and evaluate patient experience for patients on the ERAS+ programme Recognise, understand and accept the requirement to act as an advocate for patients, carers, service and organisation.
Communications Attend meetings with the Colorectal Cancer CNS, Business Group Heads of Nursing, as required.
Ensure patients receive appropriate verbal and written information and support in relation to their surgical treatment and discharge post intervention Facilitate open and effective communication within the specialist team and external organisations Ensure patient pathways are supported by appropriate documentation.
Develop links with the Clinical Network and other specialist services to facilitate the sharing of good practice as required Lead by example, utilising communication skills to ensure timely and effective communication with patients, staff, relatives and carers in relation to care issues Assist in the management of patient, relative and staff concerns, liaising with staff in the Patient and Customer Services Department as necessary Seek support and advice on matters outside experience and authority Responsibility for Patient Care Provide specialist advice, support and leadership to patients, carers and staff on issues relating to Enhanced Recovery after Surgery +.
Act as a role model at all times and contribute to the provision of nursing expertise and specialist knowledge As part of Personalised Care for Cancer, work with the Cancer Care Coordinator and Colorectal Cancer CNS team to provide Personalised Care and Support Planning (PCSP) for all cancer patients referred to the service and ensure that any actions are undertaken and communicated appropriately Promote self-management, undertaking risk stratification to determine those who can self-manage and those who require education and support to facilitate self-management Maintain accurate patient records, by collecting, collating, recording, evaluating and reporting information under the supervision of the CNS Attend the MDT meetings as required and actively contribute to the discussion, decision making and care planning for individual patients planned for surgical intervention Establish and maintain a highly effective communication with patients and carers in order to develop a therapeutic relationship within which sensitive, distressing and complex issues are often addressed.
Assess patients information needs and provide relevant information.
Assist in the promotion of patient and public experience feedback through local initiatives to drive innovation and change Ensure the use of risk assessments and health and safety management, utilising the Trusts incident reporting system, to improve the quality of patient care Encourage and promote an evidence-based culture in staff through the use of cancer specific pathways, procedures and protocols of care within a multidisciplinary approach.
Ensure that Child Protection and Vulnerable Adult legislation is adhered to.
Planning and organising Be involved in developing standards of care, practice guidelines and where appropriate Integrated Care Pathways for the patient/client group.
Contribute appropriately to clinical governance activities that relate to own area of practice and patient/client group.

Support in the management of change within a complex environment in response to emerging knowledge and national cancer initiatives Work collaboratively with other professionals and agencies to ensure that the patient and carer needs are met, ensuring the involvement of the multi-professional team as appropriate Work collaboratively as a member of the MDT, leading on issues relating to own area of specialist nursing care and management of the patient pathway, ensuring patients needs are effectively represented and addressed Actively participate in multi professional group meetings.
Assist the Colorectal Cancer CNS team in the opportunities for clinical placements and ensure quality is enhanced through supportive teaching and mentoring.
Assist in ensuring a positive approach to customer care through conflict/complaints handling Assist in ensuring the required skills and knowledge to deliver specialist care are clearly identified and adhered to within the service Responsibilities for Teaching and Training Develop, coordinate and deliver relevant teaching and educational programmes for all disciplines and grades of staff and multi professional team members throughout the Trust.
Provide advice both within and outside own profession to ensure best practice is maintained.
Prepare and present formal presentations, educate and inform on best practice where appropriate.
Maintain own professional development by attending congresses and educational meetings.
