Job Description The Care Coordinator plays a crucial role in ensuring the effective coordination and delivery of healthcare services to patients across the PCN. The specific duties and responsibilities of the Care Coordinator will vary depending on the needs of the patient population, but generally, will involve the following: Patient Care Coordination: Facilitating communication and collaboration among healthcare providers involved in a patient's care, including primary care physicians, specialists, nurses, and other allied health professionals. Care Planning: Working with the healthcare team to develop and implement care plans tailored to the individual needs of patients. This may involve assessing patients' health status, identifying goals, and creating plans that address both immediate and long-term health needs.

**Resource Navigation**: Assisting patients in accessing and navigating various healthcare and community resources, such as social services, support groups, and other relevant programs. Patient Education: Providing patients with information about their conditions, treatment plans, and self-management strategies. This helps empower patients to actively participate in their own healthcare. Monitoring and Follow-Up: Monitoring patients' progress and ensuring that they adhere to their care plans.

Following up with patients to address any concerns, answer questions, and make adjustments to the care plan as needed. Communication and Advocacy: Serving as a liaison between patients and healthcare providers, advocating for patients' needs, and ensuring that information flows smoothly among all members of the healthcare team. Population Health Management: Contributing to the management of the overall health of the population served by the primary care practice. This may involve identifying trends, coordinating preventive care measures, and addressing health disparities.

**Quality Improvement**: Participating in quality improvement initiatives to enhance the efficiency and effectiveness of care delivery within the primary care setting.
