Care Planning: Working with the healthcare team to develop and implement care plans tailored to the individual needs of patients. This may involve assessing patients' health status, identifying goals, and creating plans that address both immediate and long-term health needs.

**Resource Navigation**: Assisting patients in accessing and navigating various healthcare and community resources, such as social services, support groups, and other relevant programs. Patient Education: Providing patients with information about their conditions, treatment plans, and self-management strategies. This helps empower patients to actively participate in their own healthcare. Monitoring and Follow-Up: Monitoring patients' progress and ensuring that they adhere to their care plans.

Communication and Advocacy: Serving as a liaison between patients and healthcare providers, advocating for patients' needs, and ensuring that information flows smoothly among all members of the healthcare team.
