An interesting and exciting opportunity has arisen for a highly motivated and enthusiastic individual to join Cancer Services at Warwick Hospital in the MDT Team.

The post holder will work closely with the wider trust including clinicians, general managers, service managers, nurses, secretaries and other administrative staff.

The ideal post holder will have previous experience with working within the NHS and previous administrative experience with an understanding of cancer plans, standards and targets.

As the facilitator for multi-disciplinary cancer team meetings you will ensure that MDT meetings run effectively and that the journey of each cancer patient runs smoothly and is progressed in a seamless and timely manner. To Collate and analyse sensitive clinical information from multiple Trust data collection systems and track of each patient pathway in a timely manner identifying and escalating delays in the pathway. Support the collection, collation and input of clinical and non-clinical audit data to meet all national standards and targets. Prepare and maintain all related documentation ensuring treatment plans are recorded in the medical notes.

**Knowledge skills and experience required**:
Organise and prepare documentation for Multi-disciplinary Team (MDT) meetings liaising with a range of professionals including Physicians, Surgeons, Pathologists, Radiologists, Oncologists, medical secretaries, nursing staff and staff from other hospitals.

A willingness to cover the work of fellow Patient Pathway Facilitator colleagues during periods of absence is required.

Ensure timely notification of meetings to MDT members. Co-ordinate team diaries and organise venues and timely circulation of essential information.

To analyse cancer waiting times data and liaise with members of the MDT and Cancer Services Collaborative Facilitator to identify potential delays in treatment.

Responsible for collating all pathology material in liaison with the relevant site lead pathologist and providing reports in preparation for meetings.

Prepare typed treatment plans as discussed at the multi-disciplinary team meetings, ensuring that the consultant has confirmed the action agreed at the meeting and a copy is inserted in the patient’s health records.

In accordance with the management plans, working with the appropriate clerical staff, facilitate the next planned appointment for the patient. Monitor performance against intention specified in the treatment plan.

Interrogate data collection systems such as Lorenzo, ICE, winscribe, Soliton and other in-house and external systems to retrieve information needed to maintain a tracking process on every two-week referral, consultant upgrade and screening referrals.

Undertake administration as required for the specific tumour site teams. There will be a variety of tasks dependent upon the needs of the individual teams and their remit on specialised cancer sites.

Assist MDT team members in obtaining research data from a variety of sources including the Internet, medical and nursing journals, newsletters etc.

Ensure the provision of a high quality service. To ensure that all Trust standards and targets are maintained and monitored to improve the quality of care to all who come into contact with services provided by South Warwickshire NHS Foundation Trust.

Undertake such other tasks as required by the Cancer Data & Performance Manager in accordance with the grade and nature of the post.

Provide cancer waiting time information for the purpose of internal and external audit when required.

To attend the relevant MDT meetings and complete appropriate data collection at the meeting, in particular ensuring accurate recording of clinical decisions. To actively participate in the meeting to ensure that defined National Cancer Waiting Time tar
