This new role for Warwick Hospital is working with the Palliative Care Team to support and enable excellence in End of Life Care for patients and families in our care.

The role will work in the Palliative Care Team and will be responsible for the further development of end of life care education for the multidisciplinary teams. You will working alongside ward teams to role model outstanding end of life care, leading with courage and compassion. You will work with the team to develop service improvement including improving recognition of dying.

We are looking for someone able to work autonomously and with excellent communication skills. You will have led service developments or projects and be confident in the delivery of education

To provide support, facilitation and leadership to all staff throughout the trust on all aspects of end of life care to improve the quality of end of life care across Warwick Hospital.

Work collaboratively with the all teams and partners in Palliative Care across service boundaries to drive best practice which enables personalised end of life care which is both measurable and achievable

Ensure on-going training and project management to embed the end of life care plans/tools and associated documentation throughout the trust (e.g. AMBER Care Bundle, Individualised Plan of Care for the Dying Person, Advance Care Planning/RESPECT, Rapid Discharge Home to Die process, Electronic Palliative Care Coordination System)

To develop an overarching education programme in partnership with the Specialist Palliative Care Team and Learning and Development Team, requiring regular adaptation and formulation of plans.

Provide education, clinical leadership and direct staff support in order to secure and maintain the highest standards of holistic care for patients entering the end phase of their life including their families/carers.

Provides best practice advice in a clinical setting for staff in relation to clinical practice and governance for The End of Life Care Service

Provide complex information to various parties including colleagues. Showcase negotiation and persuasion skills in communicating educational detail for the end of life care service.

Assesses clinical interventions and assesses, benchmarks clinical education, supports mentors/educators in complex decisions relating to student placement, competence and fitness for practice.

Lead the Companions for the Dying Programme of volunteer support

To undertake/ensure participation in audits both locally and nationally, provide reports and support the delivery of learning to improve practice as necessary.

Gather appropriate data to inform service outcomes and outcome measurement

Facilitate quality improvements in the provision of end of life care across the trust and build a sustainable systematic model for the future.

To be a visible role model, providing clinical support and practical guidance on all aspects of end of life care to the wards.

Lead the trust’s end of life care nurse champion/link nurse initiative.

Responsible for development of mentorship programmes and learning environment in line with education policy.

Ensure end of life care standard operating procedures are produced, disseminated and updated as required. Comments upon and assists in implementing policies and procedures for the End of Life Care Services.
