**The Transformational Lead**: Palliative and end of life care role in partnership with Macmillan is an exciting opportunity for anyone wanting to further develop their career in system level strategic transformation, and has a special interest in palliative and end of life care. The post is part of the Coventry & Warwickshire ICB Clinical Transformation Team and will be supported both regionally and nationally by a new Macmillan network.

The post holder will lead the Coventry & Warwickshire Transformational Palliative and End of Life Care programme, with responsibility for the development of the Palliative and End of Life Care Strategy: Delivery Plan.

This role is in conjunction with Macmillan, which will enable the post holder to be part of a new Macmillan transformation programme which will provide leadership development and connect the post holder with a UK wide community of practice. The highly individualised programme of personal and professional development is designed to provide transformational leaders with the specialist knowledge, skills and behaviours required to transform and deliver the highest quality services.

This is a new and exciting opportunity for an experienced leader to develop and deliver the Coventry and Warwickshire Palliative and End of Life Care transformation programme. The post holder will provide transformation skills, leadership and knowledge to drive a high quality PEoLC programme across the Coventry & Warwickshire footprint.

To be responsible for the delivery of the PEoLC transformation programme across the health and social care system in Coventry and Warwickshire

To be responsible for the planning, delivery and oversight of multiple commissioning and re-design projects and programmes using formal project management approaches, evidence-based service improvement methodologies and tools to support implementation, monitoring and assurance.

As part of the clinical transformation team the post holder will be responsible for leading the management of a schedule of projects which will include:
Progressing integrated transformation models across a range of provision,

Service re-design activity linked to commissioning intentions, local need and system priorities,

Initiatives linked to reducing health inequalities,

Self-directed support.

The post holder's main responsibilities among others will include:
Responsible for leading and managing the PEoLC transformation programme and work streams on behalf of the Clinical Transformation team and ICS, co-ordinating activity to deliver agreed ICB and local authority objectives within required timeframes and budget.

Responsible for developing and nurturing key relationships and networks including local, regional and national networks in order to deliver the pace and magnitude of transformation and change required.

This role supports the clinical transformation team within the ICB Nursing and Quality Directorate, in driving clinical transformation, improving care, and ensuring barriers are being tacked in the health inequalities which people with palliative and end of life care needs and their carers face.

To support the Senior Responsible Officer (SRO) for PEoLC transformation as required.

To chair meetings and be responsible for the delivery of project work with multiple providers of services including acute, community, mental health, local authority, independent, third sector and communities. Projects will involve managing and interpreting highly complex sensitive information and sharing at senior and strategic level.

Identifying and proactively managing conflicting complex priorities within the organisation and across the partnership with significant inter
- dependencies.

Responsible for leading the innovative implementation and achievement of agreed strategic intentions for clinical transformation projects as set out in commissioning plans and strategies.

Responsible for ensuring transformation programmes are delivered in a timely and appropriate way through the project life cycle from analysis, scoping, development of business cases, planning, implementation and review.

Work effectively in a complex matrixed approach with ICS colleagues and partners to design and deliver key work programmes that meet the objectives of the ICB and partnership, including supporting
