Patient Safety Facilitator Midwife

The post holder will be responsible for leading and organising the weekly incident meeting, encouraging a full multidisciplinary attendance and sharing all learning highlighted with the staff. The post holder will be responsible for allocating leads where an after-action review is required and ensuring timelines are adhered to. They will work closely and collaboratively with the Patient Safety Lead Midwife and wider patient safety and governance team to ensure effective communication systems are in place and to ensure excellent standards of care and compliance with national and local standards
- To ensure that all clinical incidents are reported and investigated by the appropriate professional in a timely manner
- To work with the Patient Safety Lead Midwife to ensure any action plans following incident reviews are monitored and due dates are adhered to
- To produce factual, comprehensive and timely reports when required following an incident review
- To risk assess as the clinical incidents are reported ensure a timely senior review is completed when required
- To lead the weekly incident review meeting ensuring a full Multidisciplinary attendance
- To support the learning from clinical incidents working alongside the wider Governance team ensure it is embedded within daily practice within the Better Together team
- To promote evidenced based practice
- Maintain a safe working environment
- Supervise and maintain accurate written and electronic records using the Trusts documentation systems and standards
- Responsible for reporting or escalating concerns which may arise out of a clinical investigation, and why it may effect safe patient care
- This is a role that will require you to work at both QEQM and the WHH sites - you will be required to work cross site and split your time between the sites.
- To provide support to the clinical leads with reviewing incidents in their local areas i.e.: Labour Ward and community settings
- To work clinically to meet the needs of the service when required
