**About The Social Care Practitioner Role**:

**Key Requirements**:
You will be an enthusiastic and dedicated person, with experience within Adult Social Care. You will be required to work with a variety of customers, including people with physical disabilities, sensory requirements, carers and their families and other professionals.
You will be required to undertake outcome focused assessments, support planning and reviews. Duty work may be undertaken within the role. Focusing on strengths and utilising an asset-based approach, you will enable customers to identify outcomes, meet their full potential and maximise their independence.

**Vacancy specific services**:
New Living and Ageing Well Community Reviewing Team

Living and Ageing well are excited to announce the development of Community Review Team. The team will consist of a full time Team Manager, Emily Harris and nine Social Care Practitioners.

The Social Care Practitioner - These are dynamic roles focusing on strength-based reviews, connecting people to their communities, utilising technology, and ensuring that the level of formal support is proportionate to need and the whole support package is enabling people to live as independently as possible. The work of this team will support financial and resource sustainability.

The team will be city wide but individual workers will be linked to Primary Care Network (PCN) areas (Network_map_20190703.pdf (sheffieldccg.nhs.uk). Undertaking 2-3 annual reviews (full time equivalent) each week. Members of this team will be trained in issuing low level equipment and will work in close partnership with providers working together to ensure personal outcomes are achieved. independence.

Living and Ageing Well Somewhere Else to Assess Team

We work with Adults who have been discharged from hospital to an assessment placement within nursing and residential homes. We complete Care Act assessments to determine eligibility for ongoing services and support. We also work with health colleagues to complete timely Continuing Health Care assessments. We support the person to arrange their ongoing care, support and if required accommodation options.

The role requires timely assessment work to support the person to achieve their wellbeing outcomes whilst enabling the assessment placements to be effectively utilised.

You will be supported with an induction to the team and have regular supervision and team meeting opportunities, as well as access to ongoing learning and development opportunities.

Living and Ageing Well Response and Enablement Team

The response and enablement team, work with adults within the living and ageing well service with eligible social care needs including those living with dementia of any age.

The team pick up and deal with emergency situations, resolve issues at the earliest point and focus on enablement by reducing the need to increase formal services by promoting and maintaining an individual’s independence wherever possible.

They are a city-wide service who support the living and aging well community teams working together as a single service. They focus on the principles of enabling first, promoting independence, delivering advice o
