About the Role

We are seeking a highly skilled Mental Health Practitioner to join our team at Sheffield Health & Social Care NHS Foundation Trust. As a key member of our Liaison Psychiatry Service, you will play a vital role in providing high-quality care to individuals experiencing mental health crises.

Key Responsibilities

  • Deliver evidence-based practice and work collaboratively with multidisciplinary teams to provide person-centred care.
  • Conduct thorough assessments and develop effective care plans for service users.
  • Work closely with Sheffield Teaching Hospitals, CAMHS, Social Care, and the Police to ensure seamless care pathways.
  • Provide leadership and support to colleagues, promoting a culture of regular supervision and team discussion.

Requirements

  • Registered Mental Health Nurse, Level 2 Social Worker, or Occupational Therapist with relevant experience in crisis mental health care.
  • Excellent clinical triaging and assessment skills, with the ability to work effectively with service users and their families.
  • Knowledge of PDR and supervision processes, with a commitment to the appraiser and supervisor role.
