**ROLE AND FUNCTION**:
To delivery support and prepare looked after young people for their transition from semi-independent provisions into independent living **OUTSIDE **of normal office hours (0900-1700 Monday-Friday) **WORKING EVENINGS AND WEEKENDS INCLUDING BANK HOLIDAYS.**

The main purpose of this Job is to ensure the following:

- To undertake delegated responsibilities for designated tasks.
- To deliver a supported living package based upon structured key work sessions to vulnerable young people aged 16 - 25. These sessions will aim to promote the life skills of young people and support their transition into adulthood.

**Key Responsibilities**
- You MUST have a full driver’s license and access to car.
- Manage people living semi-independently in the community.
- Manage your own timekeeping and structuring key work sessions consistent with an independent placement agreement.
- Work with other key workers and young people to develop structured key work sessions.
- Monitor the young person’s ability to engage with everyday living and develop skills where necessary.
- Promote safe, consistent, and understandable boundaries with young people in conjunction with the license agreement and any other documents such as a behaviour agreement.
- Encourage the young person to take an active part in the local community, building relationships and maintaining social networks.
- Complete Daily logs for each young person.
- Prepare monthly detailed progress reports for each young person’s progress. These reports must be completed to a high standard for submission to the placing authority.
- Work in partnership with other agencies and support the young people to attend meetings with other professionals.
- Attend admission and planning meetings; and attend LAC reviews ensuring all relevant paperwork is complete.
- Carry out weekly Health and Safety checks for each property with the assistance of the young person.
- Maintain confidentiality for young people and report any concerns regarding young person’s welfare to the Manager.
- Understand and be able to implement the policies and procedures of Elliot Leigh TLC.
- Attend staff meetings and training sessions as and when required.
- You will be expected to take part in structured supervision sessions at least once every six weeks.

**Eligibility**
- You must be eligible to live and work in the UK and provide proof of eligibility.

**Hours**
- Your hours will be flexible and determined by the number of hours allocated to the young person, and any other specific tasks you further agree to undertake.
- The times must be pre-approved by the Service Manager in advance.

**Job Type**: Part-time
Part-time hours: 2 per week

**Salary**: £18.00 per hour

- GCSE or equivalent (preferred)
