Advanced Practitioner and Clinical Lead

This is an exciting opportunity for an experienced nurse, therapist or paramedic to join a crisis response service designed to reduce preventable hospital admission. The referral source will typically be from general practice, NHS 111, A&E, ambulance services or community
- based health and social care (including care homes).

The aim of the service is to avoid unnecessary hospital admission by:

- Providing the Merton population with an alternative to hospital admission through rapid deployment of clinical response to patients in their own homes.
- Providing a rapid response (2-hour response to referral, within operating hours) to make initial assessment and form onward plan.

The current post holder works 8.00 - 16.00 Monday - Friday but there is flexibility with this as the service runs from 08.00-20.00 including weekends and bank holidays.

The team is comprised of nurses, physiotherapists and occupational therapists, with access to GPs, pharmacists, case managers, social services, dietitians and other specialist nursing and therapy services, all co-located at Merton Civic Centre.
- Experience as a non-medical prescriber, so you can support staff undertaking the V300 course and sign off their competencies.
- Be clinically responsible for the team

- Be responsible for undertaking comprehensive health assessments, physical examination, and ordering tests for patients.
- Use advanced clinical decision making and assessment skills, coordinate care planning and initiate referrals to diagnostics for appropriate tests for this population.
- You will play a leadership role within the team and work with external stakeholders such as London Ambulance Service and Social Services to develop new ways of working and through networking increase the number of referrals from the acute and community.
- Possession of a UK driving license and access to a vehicle for business purposes is essential.
- A commitment to personal and service development is essential
