Advanced Practitioner

We are seeking a highly skilled and experienced Advanced Practitioner to join our Rapid Response Team as a Team Lead. The successful candidate will be responsible for coordinating intensive and complex nursing care with other multi-disciplinary agencies, working collaboratively with health and social care professionals across the borough and Integrated Care System.
The ideal candidate will have significant previous post-registration experience in the relevant key areas at Band 6 or above, with experience of working in a community or urgent care response team. They will also have experience of managing in a customer-focused environment, providing professional support within a busy environment.
Main Responsibilities- Co-ordinate intensive and complex nursing care with other multi-disciplinary agencies.
- Work collaboratively with health and social care professionals across the borough and Integrated Care System.
- Participate and complete managerial and clinical supervision.
- Provide line management supervision to members of the Rapid Response multidisciplinary team.
- Conduct and contribute to Datix investigations/Root Cause Analysis/Serious Incidents and to help support any lessons learnt within the service.
- Support and mentor pre-registration learners from a variety of disciplines.
- Registered Nurse (Part 1 of NMC register) or Health & Care Professions Council eg Paramedic.
- Non-Medical Prescriber.
- Postgraduate level study or able to demonstrate postgraduate level working evidenced via continuous professional development.
- Certificate/Diploma in Management Studies Specialist Community Practice Teacher or equivalent.
This is a permanent position, working within the Rapid Response Team, Woodbury Unit, First Floor. The salary is £43,742 to £50,056 a year pro rata (plus HCAS).
