**Overall Purpose of the role**: The Rapid Response Service is a 24/7 service delivered across two shifts: 08:00-20:00 and 20:00- 08:00. During those hours the Rapid Response team will respond to the needs of patients identified as appropriate for the service across Trinitys catchment area by providing specialist palliative nursing and end of life care. The PCA will support patients in their own homes to live as fully as possible with their life-limiting condition. The PCA will be the eyes and ears for the Registered Professionals who hold responsibility for the patient's care.

The PCA will use excellent communication and listening skills to enhance patients' psychological wellbeing; be adept at recognising when patients' conditions change and report this to the relevant member of the specialist Multidisciplinary team. Key Tasks To support the functions of the Clinical Hub including: Specialist Community Nursing To provide personal care within any existing care plans, and in accordance with the Clinical Nurse Specialist, and medical care prescribed by the GP. To provide personal care and support to the families and carers of individual patients in the place they call home across the Trinity catchment area, being sensitive to their changing physical, psychosocial and spiritual needs. To communicate and liaise with the patients usual care team including District Nurses, other palliative care support such as the 111 Roaming Service as well as the Clinical Hub Team Lead, reporting on the condition and needs of the patients and providing and maintaining clinical records, as necessary.
