Care Coordinator

This role is mainly office based and will be to provide support to staff and service users who are based in the community. If needed you may be required to work in the community providing support to vulnerable adults outside of your normal working hours.
Completing staff rotas - to the right skills mix - in rounds and in geographical order
Monitoring service users care delivery to ensure that they receive a high level of service through - call monitoring - monitoring time sheets - monitoring online notes and EMARs
Recruiting care workers - Interviewing - HR - supervision and monitoring staff performance
Providing council reports
Taking on call duty phone on a rotation basis
Service user assessments care plans and reviews

Qualified Care worker to NVQ/Diploma Level
Managing and supporting staff
Experience in direct care delivery
Experience in supporting staff and a knowledge of employment law
IT Skills
Excellent customer service skills
A desire to provide the highest quality of care within Wakefield

Driver with own vehicle
Flexible in working hours due to the nature of the role.
