Here is an exciting opportunity to join our Therapy team at Broomfiles as a Day Therapies Patient Coordinator.

The role of the Day Therapy patient coordinator is to provide specialist knowledge for the pathway of Day Therapy patients across Mid and South Essex NHS Foundation Trust. You will facilitate the smooth running of the patient journey by ensuring that the pathway is co-ordinated, streamlined and all processes revolve around the patient and their individual needs.

You will work directly and indirectly with patients and their carers, and be responsible for capturing information about patients, ensuring that up-to-date relevant information is recorded on the patient record and Trust systems. You will have a good knowledge and understanding of the specific patient pathways. As a key requirement of this role, the post holder will have excellent communication and organisational skills, together with a high attention to detail and levels of accuracy.

The role will involve supporting and working within the clinical nurse teams, pharmacy and the receptionist/ward clerk to ensure that the process is managed in a timely manner up to the point that the patient attends for their appointment. YOu will be responsible for backfilling the ward clerk during periods of absence including annual leave.

Liaise with Consultants, Doctors, Clinical Nurse Specialists, Nurses, Pharmacists, and receptionist / ward clerk, providing advice, information and guidance to patients, carers and relatives about appointments, prescribed treatments, clinics and admissions.

Assists by pulling patient packs twice a week in readiness for unit pharmacist.

Occasionally book transport for the patient if they are unable to do this themselves.

Liaise with a range of stakeholders across the organization.

Exchange information with patients, relatives and staff on a variety of departmental matters and procedures, anxious patients and relatives, cultural or language difficulties.

Relay information in a tactful and sensitive manner between the referring departments.

Monitor patient attendance of appointments and re-arrange/book further appointments as required.

To ensure appointments are booked taking into account of situations such as surgery dates, patient preference, unit capacity and /or dependent on blood results, otherwise chronologically and in accordance with National and Local Guidelines.

Co-ordinate, monitor and initiate appropriate changes along the patient pathway to ensure the patient’s journey is managed efficiently, smoothly and in accordance with targets and agreed timescales.

Support the implementation of changes to the patient’s treatment

Liaise with patients

Utilise computerised tracking systems to locate case notes and investigation results.

Liaise with teams outside of the speciality e.g., G.P’s where appropriate to ensure a seamless patient journey.To identify and resolve delays in the patient pathway. Where this is not possible, to follow the escalation procedure, highlighting any relevant issues to the line management.To provide cover for absent colleagues to ensure a smooth-running service, including the booking of appointments.To develop a good working knowledge of anatomy, terminology, drugs used, treatment types, medical coding and pathology relating to the specialties involved.To liaise with other Trusts both within the Network and outside, in order to achieve continuous monitoring of the patient pathway.To identify and advise the clinical teams on problems with the pathway including panel agreement of drugs, funding issues working closely with pharmacy. When a delay to treatment occurs to the patient journey, the post-holder ensures that these are accurately documented and if necessary, communicates with the patient ensuring sensitivity and tactfulness is upheld.T
