An opportunity has arisen for a Patient Flow Coordinator to join our friendly team working on Lister Ward, a 25 bedded Acute Medical ward at Broomfield Hospital.

The Patient Flow Coordinator role provids the opportunity to make a difference to patient experience and the end of their journey within the hospital. The successful applicant will be required to work with patients, families and staff to support a smooth discharge from hospital. The Patient Flow Coordinator will liaise with all members of the multi-disciplinary team to help co-ordinate services and solutions required for discharge.

Hours of work will be 07:00 - 15:00 Monday to Friday.

You will establish a referral system for patients prior to admission (elective patients) who will require discharge planning.

You will actively engage with patients’ integrated care case managers / navigators where an allocation has been identified and to work within an established communication system with the community single point of access in relation to advising GP’s with regard to un-scheduled admissions.

You will work within an established referral system from wards to the discharge team for emergency/tertiary admission requiring discharge management.

Maintain a detailed list of the inpatient case load. This should include: Delayed Transfers of Care (DTOC), repatriations and predicted complex discharges.

You are also required to work with the Senior Patient Flow Coordinator to ensure referrals to appropriate services are correctly made to ensure safe discharge dependent on patient needs.

Use own analytical skills by liaising with the multidisciplinary team to ensure the correct discharge pathway is selected for the patient.

Ensure early identification and referral of patients needing repatriation to other hospitals.

Answer the phone and deal with requests for information, whilst maintaining patient confidentiality.
