**Job description**

Hands on Care is a non-profit organization that provides short-term rehabilitation services to adults with a wide range of physical, mental, and emotional challenges. We provide a variety of services including:
**Assist with activities of daily living (ADLs), personal care, and socialization.**
- Perform routine medical procedures.
- Provide transportation for individuals to and from treatment.
- Provide support for the individual during the physical and emotional journey of recovery.
- Provide support for the individual’s family (providing transportation or arranging for care)
- Participate in planning and implementation of individualized, personalized care plans.
- Participate in planning and implementation of group treatment plans.
- Participate in planning and implementation of community treatment plans.
- Participate in planning and implementation of school-based treatment programs.
- Participate in planning and implementation of special needs treatment programs.
- Assist individuals with skills related to job training and employment preparation.
- Provide support for individuals with physical disabilities resulting from injuries or illness.

Qualifications:
**Must have a valid driver’s license and automobile insurance with a good driving record.**
- Must be 21 years of age or older.

**Job Types**: Full-time, Part-time, Permanent
Part-time hours: 16-45 per week

**Salary**: £10.50-£11.50 per hour
