Wakefield
We are looking for a Family Support worker to deliver high quality practices and deliver the best possible outcomes for the client and families.
**What you will do**:
The post holder is required to deliver one to one support, working in depth with families and individuals in the community. The work involves home visits, practical and emotional support, advocating on behalf of families, making referrals to other services, telephone support and facilitation of groups and activities
