Position**:Support Worker**

(You may have been called a ‘care worker’ elsewhere) We are offering new and exciting positions for Support Worker at AV Support, based in Wakefield.

**Function**:
We provide supported living to adults who are vulnerable but wish to remain living in the community. The focus of the Company is to improve people’s independence skills and support them in the future.

The purpose of the role is to provide support to clients with various vulnerabilities & daily challenges. The support provided by Support Worker will enable people to live as independently as is possible given their particular needs.

**Location**:
Dependent on where the person being supported lives

**Responsible to**: Registered Manager, Team Leaders

**Responsible for**: Junior staff on occasions as directed by the line manager

**Hours**: To be confirmed at interview

**Salary**: Minimum rate per hour throughout probation, plus travel time & mileage, plus enhancements. Salary may alter thereafter, based on abilities & aptitude.

**Objectives of the Role**:
As a Support Worker, you will be supporting, motivating and empowering the person being supported to improve their independence skills whilst they live in their home environment. This may include developing their basic life skills abilities, enhancing their cooking & domestic skills, supporting them in learning how to deal appropriately with their finances, encouraging them to attend college sessions or undertake work. Dependent on needs, you may be required to help them with personal care, or with prompts.

**OUTLINE OF RESPONSIBILITES**:

- To have a keen interest in caring for individuals who are vulnerable and require support from others. This may also include supporting someone who has daily challenges. It may also involve being able to de-escalate any given situation and to keep focus on the person’s achievable goals.
- To support each person as they require and according to their support plan; to maintain up to date progress records, which the supported person agrees with.
- To provide support, as defined in their support plans, in a person centred way to enable them to lead as independent a lifestyle as is compatible with their needs and in accordance with their aspirations.
- Using Person Centred Planning, develop and maintain support based on the identified needs and individual aspirations of each person. Encourage the person being supported to consider opportunities in the community, helping them to maximise their full potential.
- To work to ensure compliance with all regulatory requirements.
- To demonstrate good communication skills, both written and verbal. To demonstrate excellent listening skills and to provide a non-judgemental level of high quality support at all times.
- Where the property is owned by an independent landlord, ensure the supported person is also supported to maintain their tenancy agreements and that the property remains health and safety compliant.
- To follow guidance from the Registered Manager & Team Leaders (s), who will have liaised with Care Managers and other professionals to identify the changing needs of person being supported and to update the support plan accordingly.
- Carry out risk assessments and support plans are in place for each supported person and add any additional identified hazards as they arise.
- To promote a positive risk between the expressed wishes and choices of the person being supported whilst ensuring that they don’t expose themselves to uncalculated risks or harm.
- To provide or find information, advice and guidance to supported persons, or support them in accessing the appropriate services to do so. This includes matters relating to personal care, finance, good housekeeping, health, neighbours, safety, security and daily work and leisure activities and ensure that this is done in a way that respects dignity, privacy and choice.
- To respond flexibly to emergencies, this may involve arranging appropriate assistance or attending to problems out of hours etc., for which you will be paid.
- Attend staff meetings to ensure participation in forward planning and continuity of Services and Quality Standards.
- To comply with Company policies and procedures as indicated in the staff employee handbooks and other Company literature.
- To ensure that you are familiar and working within Company policies and procedures as standard working practice.
- To work towards the achievement of organisational targets determined by the requirements of our external contracts.
- To develop and demonstrate experience in the design and delivery of supported life skills training to the people we support.
- To demonstrate knowledge of how to communicate well with persons supported who have communication issues and/or learning abilities of all levels and protected characteristics.
