- Coordinate and manage the care of patients, ensuring they receive appropriate services and support
- Collaborate with healthcare professionals to develop individualized care plans
- Schedule appointments and arrange transportation for patients as needed
- Communicate with patients, families, and caregivers to provide information and answer questions
- Maintain accurate and up-to-date patient records
- Monitor patient progress and make adjustments to care plans as necessary
- Assist with insurance verification and authorization processes
- Provide education and resources to patients and their families

- Previous experience in a healthcare or social services setting preferred
- Knowledge of medical terminology and healthcare procedures
- Proficient in Microsoft Office Suite (Word, Excel, Outlook)
- Excellent communication skills, both written and verbal
- Strong organizational skills with the ability to multitask and prioritize
- Ability to work independently as well as part of a team

Pay: Up to £11.50 per hour

Expected hours: 37.5 per week
