About the Role:

We are seeking a skilled and compassionate Female Care Coordinator to join our team in Plymouth. As a Female Care Coordinator, you will play a vital role in coordinating care and support services for individuals with learning disabilities in their own home.

Your primary responsibility will be to develop and implement individualized care plans, ensuring that each individual receives the necessary support to maintain their independence and achieve their goals.

You will work closely with a multidisciplinary team, including healthcare professionals, social workers, and family members, to provide comprehensive support and address any challenges that may arise.

Key Responsibilities:

  • To participate in the day-to-day implementation of individual care plans and activities within a challenging environment.
  • To maintain effective communication and liaison with service users, family, and others while respecting appropriate confidentiality.
  • To coordinate care and support services, including meal preparation, laundry duties, cleaning, and social events.
  • To maintain accurate notes and records as required.
  • To work as part of a team to provide high-quality care.

Requirements:

To be successful in this role, you will need to possess excellent organizational and time management skills, with the ability to prioritize tasks and manage multiple projects simultaneously.

A diploma in health and social care or willingness to work towards one is essential. Ideally, you will have experience working in a care setting, but this is not essential.

You will also need to demonstrate the ability to remain calm under pressure and work effectively as part of a team.
