To establish and maintain systems and processes to monitor and report on the delivery of this growing service. To respond to telephone and face to face enquiries from patients, Trust staff, Homecare Companies and Commissioners. To redirect and prioritise queries as appropriate, ensuring both efficiency and effectiveness of service delivery. To work with suppliers to resolve any patient-specific queries or process issues.

To ensure repeat prescription requests are processed effectively. To establish and maintain databases of patients receiving drugs recharged to Commissioners to enable effective on-going monitoring of patients as well as enabling both clinical and managerial audit. To ensure correspondence with GPs and other medical professionals, both within and outside the Trust, is up-to-date. To prioritise and manage own workload without direct supervision and to respond and react effectively to situations as they arise.

To provide a high quality administrative service to ensure efficiency in patient care for patients undertaking Biologic Treatment. To manage all patient files, ensuring information is accurate and up-to-date, and to ensure tracking of patient files as per Trust policy. To devise and maintain a database of patients on High Cost Drugs in Dermatology. To undertake additional ad-hoc duties as reasonably required by the Trust.

Demonstrate a broad understanding of pathways and treatments offered within Kingston Hospital NHS Foundation Trust and tertiary referral centres. Attend High Cost Drugs MDT meetings and provide assistance to the Nurse Consultant /CNS with arranging tests, investigations and outpatient appointments. Refer to other team members when appropriate and following discussion with the Nurse Consultant /Clinical Nurse Specialist. Develop good working relationships and liaison between other departments and Healthcare professionals within the Kingston Hospital Participate in initiatives to explore patient experience and service feedback as well as supporting clinical audits.

Support the clinicians by submitting assessment questionnaires to patients prior to their appointment. Under the guidance and supervision of the clinical team, to monitor the booking and completion of patients investigations (e.g. blood tests, x-rays, TB assessment,). Communications: 12 Ensure patients receive timely and appropriate information in regard to their treatment and signpost to appropriate supportive leaflets and literature.

13 Respond to telephone queries by a model of triage and appropriate refer to Nurse Consultant / CNS or other allied healthcare professional 14 Communicate directly with patients, relatives and carers demonstrating empathy, understanding and respect. Information discussed and received will often require tact or persuasive skills, to ensure safety and understanding around treatment side effects as well as an understanding of complex information around treatment options. Patient Information: Coordinate access to patient information and education resources for patients. 25 Undertake a focused mapping of the range of information / education support resources available in the local health and social care sectors.

Support the delivery of information prescription delivery in consultation with health professionals and according to any agreed information pathways, including printing of information. Collaborate and work closely with local providers of validated information resources, users and user groups. Human Resources (if appropriate) 28 Support colleagues, demonstrating own activities and providing advice as necessary. 29 Demonstrate excellent interpersonal and communication skills 30 Share knowledge from training days attended and actively participate and contribute to the Practice Development portfolio within Dermatology.
