This role is working within the Intensive Support Function team. The team works with young people who have a diagnosis of Autism and/or a Learning Disability who are at risk of a hospital admission and/or placement breakdown.

The team are an enhanced service to the 0-18 Mental Health teams and work closely with wider services to support these young people. The team implement the Positive Behaviour Support model and work closely with families/carers and educational settings to implement proactive strategies in order to improve the young persons quality of life.

Within this role you will be required to support a Positive Behavioural Support Pathway involving the completion of participant and non participant observations, functional assessments and the implementation of plans and interventions.

CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.
