We are looking for an experienced, enthusiastic, innovative and self-motivated Heart Failure Specialist Nurse to join our team as the Clinical Lead for our Heart Failure Service. This role presents an exciting opportunity to work as part of this interagency, multidisciplinary team to support patients living with heart failure in the local community. The role includes all aspects of heart failure care providing highly specialist clinics, education, training and clinical advice for the heart failure MDT and stakeholders across care settings. As a senior member of the heart failure team, you will be expected to lead on service development including audit, writing/reviewing policies and guidelines and participate in research.

The successful applicant will have significant post registration qualifications and experience within heart failure/cardiology, including non-medical prescribing and advanced physical assessment skills. You will take a lead role in the advanced clinical assessment and treatment for a wide range of a people living in the community with a diagnosis of heart failure, practising autonomously to provide patient-centred clinical care. You will also be responsible for ensuring that evidence-based practice is inherent in all aspects of care and treatment, sharing knowledge and best practice throughout the clinical area, Trust and externally as appropriate.

As part of our wider Long Term Conditions Team, the heart failure service has close working relationships across the Staffordshire and Stoke on Trent Integrated Care System, including primary care, acute hospitals and social care teams.

You will provide expert clinical leadership to a team of Heart Failure Specialist Nurses and Assistant Practitioners; working alongside Operational Leaders you will have an integral role in the development of the service, through collaboration with colleagues from across the local healthcare system. The ideal applicant will have a Masters award in Advanced Clinical Practice, or a willingness to work towards, and be able to demonstrate a high level of clinical expertise and leadership that encompasses the four pillars of clinical practice, leadership/management, education and research. You will need to have a commitment to professional development and contribute to a culture of continuous improvement, along with experience in leading, supporting and supervising colleagues/a team.
