We are seeking a highly motivated, person centred individual with excellent clinical leadership, management and communication skills, to job share with our existing Clinical Team Lead for the CLDT in South Staffordshire. We are looking for someone who is enthusiastic, dynamic, caring, compassionate and fully committed to multi-disciplinary working.

This Clinical Lead role, offers the opportunity to help lead the delivery of effective, responsive and person centered care for people with learning disabilities with complex physical and/or mental health needs in the beautiful county of Staffordshire.

The CLDT provides specialist multi-disciplinary support in the community for people who have learning disabilities. The successful applicant will have experience of working with people with learning disabilities who have complex needs; including needs relating to their physical health, mental wellbeing, autism and whose behaviour challenges services.

To provide clinical leadership and clinical advice to staff working within the CLDT’s and to provide clinical leadership within the CLDT.

To support nurses in complex case management and ensure the provision of appropriate clinical and managerial supervision and to provide operational supervision none nursing members of the Community Learning Disability Team

To ensure there is accountability regarding the community caseload and current waiting list. To liaise closely with Clinical Nurse Specialists to provide seamless service across the Specialist LD directorate.

**KEY DUTIES AND RESPONSIBILITIES**:
Provide clinical leadership and clinical advice providing a visible point of call for staff to go to regarding clinical and operational issues.

To provide support with operational supervision to members of the community learning disability team.

To ensure there is accountability regarding the community caseload and current waiting list.

To help define the role and parameters around community nursing.

To support the team in complex case management.

To provide a constructive clinical lead to ensure that MDT decision making is in line with clinical.

Processes i.e. Duty and Intake ensuring all service users referred have a health coordinator.

To manage clinical and operational performance issues for the CLDT and work supportively with other Professional leads cross the Directorate to ensure a seamless service.

To assist the team to implement clinical processes in line with the Clinical Strategy.

To lead on clinical audit and service improvement as required for the CLDT.

Work closely with the Head of Nursing and other professional leads to address and progress any issues highlighted.

To ensure the quality of care received by people with learning disabilities is appropriate to meet their needs highlighting gaps where necessary.

To possess a good working knowledge of the Mental Health Act 1983, and its revisions and amendments in working practice, considering Responsible Commissioner Guidance.

To possess a good working knowledge of the Mental Capacity Act, including DOLS and its interface with the Mental Health Act 1983.

To support the implementation of the CPA approach within the directorate and attend reviews with other professionals from within health & social care to ensure health needs of service users are identified and met appropriately.
