You will also support the tracking and logging of caller referrals to enable the consistent follow up of patients in a timely manner in line with Pharmacy standards.

**The main responsibilities of this position include**:
Manage the referral by entering and checking clinical records, details, and demographics, amending and updating when required.

Refer all referrals to the right appropriate member of the pharmacy team.

To take excellent quality meeting minutes at relevant pharmacy meetings. To provide

timely draft and re-drafts of minutes to meeting Chair(s) prior to approval and Group dissemination.

Disseminate relevant pharmacy circulars and memos on instruction.

Check the Pharmacy Inbox at least daily and forward queries or questions outside of own parameters to the most relevant pharmacy staff member.

Accurate and appropriate information is recorded within the service user’s clinical notes and/or using the Trust’s clinical information systems.

To contribute to audits concerned with medicines management and pharmacy service -, this will include collecting and modifying data and carrying out questionnaires / surveys where necessary and interrogating and analysing results for reporting purposes.
