We are looking for an enthusiastic Practice Consultant to join the Eastern Locality Team. A qualified Social Worker with at least two year's post-qualifying experience, you will have the ability to work under pressure; have strong organisational skills; be passionate about making a difference; and ready to embrace change.

You will have the ability to provide the specialist knowledge needed to support the management of complex cases, including safeguarding. Approachable, you will support our team of Assistant Practitioners, Social Workers and Occupational Therapists taking an active role in their practice development, supervision and wellbeing support. You will take a lead role in the operations of the team including managing workflow and providing casework consultations. You will be expected to ensure practice is legally and procedurally compliant.

Working collaboratively with partners and providers is key to this role as is the development of staff at all levels. Our priorities include developing team and individual performance; delivering person-centred outcomes based on strengths-based practice; and promoting NCC priorities like Living Well and better integrated care.

**Integrated Community Teams **Care incorporating Older People and People with Physical Disabilities

Our teams work with older people and adults with a physical disability. Our teams of social workers, assistant practitioners, Occupational Therapists and reablement staff from Norfolk First Response work across all five localities. In the east we work closely with colleagues from East Coast Community Health Care. Our senior management teams are made up of Operational Directors and Heads of Integrated Care who either work for Norfolk County Council or Norfolk Community Health & Care (NCHC). It’s a really integrated environment.

We work as part of the Integrated Care System and are also aligned to Primary Care Networks which means the integrated team structures benefits hugely from great collaboration with primary care colleagues. Our teams are strong voices for social care and social work within our integrated model and ethos of prevent, reduce and delay. The teams promote a supportive and learning culture with reflective practice and other learning opportunities such as practice teaching and social work apprenticeships encouraged.

We also have an Integrated Home First Hub of integrated health and social care staff which facilitates discharges from hospital. Our dedicated team facilitates discharges from Norfolk acute, mental health and community hospitals. We are also part of the Norfolk Emergency and Admission Avoidance Team (NEAT) which is an integrated team with health colleagues to prevent admissions to hospital.

Large swathes of the Broads National Park run from north to south, with windmills dotting the skyline and huge beaches forming the coastline, including the seaside towns of Hemsby, Caister-on-Sea and Southwold. Inland, Beccles and Bungay are beautiful historic market towns.

The largest Norfolk town in this locality is the popular seaside resort, market town and maritime port of Great Yarmouth, 21 miles east of Norwich and easily accessible by road or rail. Lowestoft in Suffolk is the most easterly town in England, 28 miles from Norwich. It is also a seaside resort, market town and maritime port with direct rail access to Norwich and also
