**Contract Type**: 12 Months with possible extension

**Hours**:Full Time, 35 hours per week (flexible working)

**Holidays**:25 days per annum + Bank Holidays

**Salary**: £27,000

**Background**

**Improving physical health care for people with severe mental illness (SMI)**

NHS England is committed to leading work to reduce the premature mortality among people living with severe mental illness (SMI).

People managing a serious mental illness are at a higher risk of poor physical health, obesity, asthma, diabetes, chronic obstructive pulmonary disease (COPD) and cardiovascular disease.

The aim of the service is to improve the uptake of Annual Physical Health Checks through an innovative train the trainer and engagement model.

**Purpose of the role**

**The main purpose of the role is the support and engagement of the Health Junction train the trainer model to increase the uptake of Annual Physical Health Checks for individuals diagnosed with a Severe Mental illness (SMI) within GP’s.**
- We are pleased to be able to advertise the new and exciting role of Physical Health Check & Engagement Worker to join our growing community support service. The Service operates a hub and spoke model to support Cheshire East & Cheshire West ICB’s and PCNs.
- This new approach to supporting Annual Physical Health Checks is based on an innovative train the trainer model developed by Health Junction.
- Part of the role includes working closely with Primary Care colleagues, Data and Service Users for the development and engagement of the train the trainer and Physical Health Checks model.
- Where necessary you may be required to undertake each of the physical health checks to support the integration of the train the trainer model within Local GP’s.

**Role Specific Key Tasks**
- Understanding the current SMI data sets within each GP.
- The ability to plan, set goals and identify priorities.
- The collection and updating of relevant data.
- To use current local systems within GP settings which include EMIS and CERNA. (training provided)
- When necessary to contact individuals who are managing an SMI condition who are eligible to attend their GP for an Annual Physical Health check.
- When required, to attend meetings with Health Junction, PCN’s and GP’s to promote and present the impact of the service.
- To identify potential opportunities for support and health promotion which may include onward referrals.
- Where appropriate you may be asked to carry out a Physical Health Check in partnership with the local GP.
- To undertake training for Health Junctions’ train the trainer and Physical Health Check model.
- To support GP’s with Annual Physical Health Checks.
- To provide and collect qualitative information including case studies and service feedback.
- Manage and prioritise a caseload when needed.
- To attend regular practice meetings when required to discuss service updates, shared learning, risk and to provide patient and practice feedback.
- Build and support GP’s who have signed up for the model.
- Use and update the case management system to collect and maintain data when necessary.
- To attend all necessary meetings with Health Junction which include Supervision, Team Meetings etc..

**Data capture**
- Use and update GPs system (EMIS) and Health Junction’s Case management system
- To collate data on the number of health checks carried out through an excel spreadsheet.
- To monitor the progress of annual physical health checks.
- To collect and update qualitative feedback.

**Professional development**
- Complete all statutory and mandatory training.
- Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.
- To attend all necessary meetings with Health Junction which include Supervision, Team Meetings etc..

**Miscellaneous**
- Work as part of the team to seek feedback, continually improve the service and contribute to business planning.
- Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner.
- Duties may vary from time to time, without changing the general character of the post or the level of responsibility.
