This is an exciting opportunity for a Band 6 experienced BACPR (Level 4) Exercise Specialist to join our Cardiac Rehabilitation team in East Cheshire.

You will be required to work multi professionally as part of the Cardiac Rehabilitation team supporting patients within their own homes and in group-based sessions within the community.

We see a range of cardiac conditions including Myocardial Infarction, Percutaneous Coronary Intervention, Coronary Artery Bypass Graft, Cardiac Valve Repair/Replace and Heart Failure. You will be involved in all aspects of the service; including functional assessments, exercise prescription, the educational component and will also take the lead with exercise as required. We are looking for a motivated, enthusiastic individual to join our dedicated team working to the highest quality standards.

As a Band 6 Exercise Specialist, you will be expected to contribute to the development and progression of the service, as well as contributing to training opportunities and innovation.

The Cardiac Rehabilitation Service is fully certified in the National Certification Programme for Cardiac Rehabilitation and strives to offer the highest service to all our patients.

**Hours per week**: 30

To perform patient functional capacity and mobility assessments. L eading individual and group exercise and education, modifying individual exercise programmes for co-morbidities and physical limitations.

To assess, monitor, evaluate and educate patients and their carers with cardiac conditions, working with other members of the team to meet the needs of patients and their families.

Adopt a flexible and dynamic approach to patient care and recognise the diverse range of CHD (coronary heart disease) and HF (heart failure) patients referred.

To contribute to the development and delivery of specialist education and training programmes for other healthcare professionals providing specialist advice where required.
