A fantastic opportunity has arisen for an enthusiastic pro-active person to work as a Discharge Planning Assistant within the Complex Discharge Team.
- We are looking for someone who is able to work on their own initiative as well as part of the great team.
- support the wards to discharge their patients in a timely manner.
- As the discharge support assistant the roll is to improve outcomes for patients being discharged once medically fit.
- Early identification of potential needs and additional support is very important in achieving a short length of stay and reduces risk of potential readmission.

An ability to demonstrate trust values and behaviors. Have relevant previous hospital experience and a desire to progress in health care.

Will have or be willing to work towards completing training that enhances knowledge and skills. Have a flexible approach to meet the needs of the service.

To working with the Complex Discharge Discharge Sisters and Matron to promote a discharge ethos amongst the wider MDT, supporting the base wards to ensure that discharges are focused, timely and safe. To focus directly on the needs of the patients requiring
support and facilitation to be discharged from the acute
hospital
- To develop close working relationships with your ward

teams, administrative support to the multi-professional team, having delegated responsibility under the direction of an appropriate registered healthcare
professional
- Act as an expert resource for patients, their carer's and the multi professional team in relation to the discharge process.
- To drive a team culture alongside colleagues working together.
- Act as an advisor regarding discharge to community settings

The Queen Elizabeth Hospital (QEH) King’s Lynn is located near some of the most beautiful scenery in the UK, along the north Norfolk coast, and not far from Sandringham House.

We provide a comprehensive range of specialist, acute, obstetrics and community-based healthcare services to around 331,000 people across west and north Norfolk, in addition to parts of Breckland, Cambridgeshire and South Lincolnshire. We have more than 4,000 staff and volunteers, approximately 530 beds, and a helipad for air ambulances. We work with neighbouring hospitals for the provision of tertiary services, including as part of regional partnership and network models of care, such as the trauma network. Some specialist services and clinics are provided in community facilities, such as the North Cambridgeshire hospital in Wisbech.

We are exceptionally proud to have recently moved out of special measures and see this as a testament to the determination of our trust wide teams to put our patients first and recognise the hard work and dedication of all our staff.

1. COMMUNICATIONS
1.1 Develop and maintain healthy working relationships with the multi-disciplinary
team, patients, carers and service users.
1.2 Contribute to the effectiveness of inter-agency working as appropriate.
1.3 Support patients and service users in maintaining their identity and relationships.
1.4 Ensure that lines of reporting with the multi-disciplinary team are timely robust
and efficient
2. KEY WORKING RELATIONSHIPS
2.1 Complex Discharge Matron, Junior Complex Discharge Sisters, Flow Coordinator, Bed Coordinator and Duty Manager
2.2 Ward Sister/ Charge Nurse, Medical Teams, Occupational and Physiotherapists, the wider multi-disciplinary team, social care and Primary care colleagues
3. DELIVERY OF EFFECTIVE HEALTH CARE WITHIN THE ORGANISATION
3.1 To support the ward manager and Nurse in Charge in maintaining patient flow
3.2 Ensure plans are agreed with patient and carers for discharge and that they are fully aware of any requirements to assist discharge
3.4 Attend board rounds, identify actions required and actively manage agreed discharge plans for patients with predicted discharge dates.( SAFER Patient Flow
Bundle/ Red2Green approach)
3.5 Liaise with the relevant team, such as Home first hub
3.6 Coordinate external transfers to other organisations such as Community Hospitals, Rehab Centres, Residential and Nursing Homes
3.7 Post holder spends the majority of their shifts on the allocated wards
3.11 Maintain effective communication with the Multidisciplinary Team regarding issues relating discharge and flow, promptly reporting areas of concern..
3.13 Carry out duties consistent with individual patient / users wishes, maintaining privacy, dignity and recognising cultural diversity and needs.
3.14 Contribute to the overall effectiveness of the care delivered by the Health care team and that benefits the patient/ user experience.
