We are looking for Registered Professionals who are self motivated, with initiative, drive, strong interpersonal, admin and IT skills to work as Clinical Co-Ordinator for West Norfolk Home First Hub.This position will be based at St James Clinic, King's Lynn, Norfolk. You will be responsible for the Triage and allocation of referrals for patients on Discharge to assess pathway, admission avoidance and early supported discharge. The Service covers 7 days a week, Monday to Friday 09.00-17.00 and Weekends 10.00-16.00 including Bank Holidays. The role will also involve integrated working with the Virtual Ward Team.
- Triage and allocation of referrals for patients on Discharge to Assess pathway, assessing the patient’s needs, determining the priority of care, processing and allocating referrals accordingly, liaising with multidisciplinary team to ensure timely responses to referrals on an individual patient needs-based basis, utilising the knowledge, skills and expertise of others in the integrated team.
- To assess and prescribe care packages for patients with long term conditions, the frail and elderly, palliative care and rehabilitation needs for example, to achieve quality of life and independence where possible.
- To work within the integrated team to facilitate early discharge from hospital.
- To work within the integrated team to prevent unnecessary admission to hospital.
- To work, liaise and communicate with all health care professionals, and statutory / non statutory agencies to provide a seamless, integrated service to our service users on the Discharge to Assess pathway and in community teams.

**The post holder will**:

- Undertake comprehensive health and social care assessments of patients including those with a complex presentation, using investigative, analytical and clinical reasoning skills and formulate a graded treatment plan.
- Formulate and execute individualised treatment plans of patients including those with a complex presentation, in collaboration with the integrated team, other agencies and the patient, utilising a range of treatment skills.
- Be aware of Assistive Technology and make referrals for this as required.
- Assess patients’ holistic needs, communicating complex and sensitive information to patients and carers as to their assessment or diagnosis, prognosis and treatment plan.
- Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance.
- Be required to use tact and persuasive skills in order to gain the patient’s cooperation in their treatment plan.
- Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care coordinated appropriately.
- To participate in multidisciplinary / multiagency meetings as appropriate, e.g.: Gold Standard Framework.
- Be responsible for ensuring workload for self and others is planned and prioritised according to service targets and needs.
- Ensure patient held records are completed for each intervention and visit, and that SystmOne documentation is completed to maintain clinical information and activity data.
- Plan and organise specialist services within the integrated team.
- To use expert knowledge of the care act to ensure compliance in relation to discharge planning.
- Ensure informed consent is obtained prior to initiating interventions.
- Demonstrate proficient IT and keyboard skills.
