**Permanent Contract | £27,349 to £29,439 per annum (Scale G) | 37 Hours per week | St James Health Centre, King’s Lynn
This is a full-time position that is office based in King’s Lynn. However, there is the opportunity to work remotely for part of the week at times when in agreement with the Practice Consultant.

The role is as follows:
The hospital refers via the Integrated HUB which comprises of professionals from Health and a Practice Consultant (PC) from Social Care.

The PC would be your immediate line manager and supervisor.

The referrals known as TOC’s (transfer of care forms) provide a description, not prescription, of the persons needs that are progressed to support discharge.

Norfolk County Council support all patients who are discharged under pathway 1. That is those who are to be discharged home with a package of care.

It is the role of the Assistant Practitioner (AP) within the HUB to pick up the TOC’s (this may change in the future) and progress by putting them onto the system, to send a request to our brokerage service who will look to source the care.

There are occasions when the HUB AP is asked to source a placement, the PC would lead and advise on those.

Generally, there is little direct or indirect communication with the service user/patient but not exclusively. There may be times when the AP needs to contact families/the person/NOK to advise for the financial policy etc. Also, sometimes the need to contact the hospital to gather additional information or for clarification.

The hours are currently 9 until 5 but the vision is for a longer working day (times to be confirmed), this would be covered on a rota within the team.

Currently there is no weekend working but this will be required in the near future. APs in the HUB would work on a rota basis and have 2 days off in the week after they have worked the weekend.

Bank Holiday cover is also a requirement within the role.
