Working under the direction of a relevant health practitioner, participate in the initial assessment of individuals with learning disabilities referred to the service who may have complex needs which may include additional physical disabilities, mental health problems, autism and/or challenging behaviours. Working under the direction of a relevant health practitioner, implement and support the development of care plans, risk assessments, intervention plans and guidelines. To participate in capacity assessments alongside members of the MDT and under direction of a health practitioner. Requires working knowledge of Mental Capacity Act and use of appropriate MCA documentation.

Under direction of a relevant health practitioner, be responsible for participating in the implementation of a range of health interventions for individuals. This may require partnership working with carers, families and other professionals. Work in partnership with social care colleagues to support the delivery of social care interventions. This may include providing advice and support to adults with a learning disability on housing, finances, care packages, etc.

Alert the practitioner and/or line manager to any changes to an individuals health, psychological or social circumstances, identifying and making use of safeguarding processes as required. Requires a working knowledge of safeguarding processes/policy. Support the therapy team in the planning, coordination, and delivery of therapy sessions. To participate in risk assessment processes and adhere to care plans and risk assessments in place.

To communicate in a skilled and sensitive manner, keeping individuals and their representatives (where appropriate) informed, maintaining confidentiality. To maintain accurate records. To work as part of the Disability Team contributing to the effective sharing of written, electronic and verbal information to ensure the delivery of a co-ordinated multi-disciplinary service. To communicate effectively with clients and carers using a range of verbal and non-verbal communication tools to gain an understanding and consent with treatment programmes.

This may include individuals who may have difficulty understanding or communicating e.g. sensory, cognitive, learning and language difficulties. To establish therapeutic relationships with clients and carers using negotiating and reasoning skills to facilitate their participation in the therapy process. To maintain accurate and comprehensive client treatment records in line with NHS Bolton Policies, Council Policies and Disability Team Guidelines.

To support the development of service documentation, including accessible letters, accessible guidelines and accessible leaflets.
