We need a talented and empathetic individual with excellent communication skills to be part of a growing team. The successful person will work across multiple surgeries, supporting people who have long-term conditions, are lonely or isolated, or who have complex social needs which affect their wellbeing.

This is a non-clinical role, but one that supports patients to address and tackle issues that are having an adverse effect on their mental and physical health such as housing, debt or social isolation.

Social prescribing empowers people to take control of their health and wellbeing through the support of non-medical link worker. The focus is holistic approach, connecting people to community and voluntary sector groups and statutory services for practical emotional support. Social prescribing can help to strengthen community and personal resilience, reducing health inequalities by addressing wider barriers and increasing peoples active involvement in local communities.

We are looking for someone with relevant qualification, skills and resilience to work with GPs and practice staff, helping people by addressing their needs in a non-medical way.

**Main duties**:
Cambridge Northern Villages PCN (Primary Care Network) is looking to recruit a Social Prescriber to join our PCN team.

Social prescribing Link worker can help to strengthen communities and personal resilience, reducing health inequalities by addressing the wider determinant of health such as debt, poor housing and physical inactivity, by increasing people’s active involvement with their local communities.

Social prescribing is particularly effective for people living with long term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which may affect their wellbeing.

This is a non-clinical role, but one that supports patients to address and tackle issues that are having an adverse effect on their mental and physical health such as housing, debt or social isolation.
- To take referrals from a wide range of agencies (including by not exhaustive)

- To provide personalised support to individuals, their families and carers to take control of their wellbeing, live independently and improve their health outcomes.
- To develop trusting relationships by giving people time and focus on what matters to me.
- To manage and prioritise you own caseload in accordance with the needs, priorities and any urgent support required by individuals on the caseload.
- To have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals or agencies, when what the person needs is beyond the scope of the Social Prescribing Link worker role.
- To work as part of a multi-disciplinary team in a patient facing role, using expert knowledge within the SPLW areas and to promote social prescribing and its role in self-management.
- To work together with all local partners to collectively ensure that local VCSE organisations and community groups are sustainable and that community assets are nurtured.
- To build relationships with key staff in GP practice within the Cambridge Northern Villages PCN, attending relevant meetings, becoming part of the wider network team, providing information and feedback on social prescribing caseload.
- To ensure that social prescribing referral codes are inputted to the clinical system and that the patient’s use of the NHS can be tracked, adhering to the data protection legislation and data sharing agreements with the CCG.
- To work in partnership with
