About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our Primary Care Network team at Cambridge City 4 PCN. As a Care Coordinator, you will play a vital role in supporting patients with complex needs, ensuring they receive timely and effective care.

Key Responsibilities
  • Work closely with patients, their families, and carers to develop and implement personalized care plans.
  • Coordinate and navigate patients through the healthcare system, ensuring seamless transitions between primary and secondary care.
  • Provide education and support to patients, empowering them to manage their health and wellbeing.
  • Collaborate with multidisciplinary teams, including GPs, social prescribing link workers, and health and wellbeing coaches.
  • Identify and address health inequalities, promoting equal access to care for all patients.
Requirements
  • GCSE grade A-C in Maths and English.
  • Ability to actively listen, empathize, and provide personalized support in a non-judgmental way.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Experience of working in a patient-facing role, preferably in a healthcare setting.
  • Knowledge of personalized care approaches and the wider determinants of health.
