Job SummaryWe are seeking an experienced Care Coordinator to join our medical office team. The successful candidate will be responsible for coordinating patient care, facilitating communication between healthcare providers, and ensuring seamless transitions between care settings.
Key Responsibilities- Coordinate patient care activities, including scheduling appointments, communicating with healthcare providers, and managing patient records.
- Facilitate communication between healthcare providers, patients, and families to ensure timely and effective care.
- Develop and implement care plans that meet the unique needs of each patient.
- Collaborate with healthcare teams to ensure continuity of care and optimal patient outcomes.
- Provide education and support to patients and their families to promote health and wellness.
- Manage and maintain accurate and up-to-date patient records, including medical history, treatment plans, and test results.
- Ensure compliance with regulatory requirements and organizational policies.
- High school diploma or equivalent required; associate's or bachelor's degree in a healthcare-related field preferred.
