Job summary
Cambridge City 4 Primary Care Network Ltd. is looking for an experienced Clinical Pharmacist to deliver and enhance the pharmacy services within the PCN. The successful candidate will join an established team of Clinical Pharmacists and Pharmacy Technicians to support and develop the services across six GP surgeries located in Central Cambridge, having a combined registered list size of 59,000 patients.
Working as part of a multi-disciplinary team in a patient-facing role the CP will take responsibility for areas of chronic disease management and clinical medication reviews to proactively manage patients with complex polypharmacy. The CP will provide primary support to general practice staff with regards to prescription and medication queries and will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care. Developing and promoting systems for safer prescribing is essential for this role alongside providing expertise in clinical medicines advice while addressing both public and social care needs of patients in our GP practices.
The CP will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.
Main duties of the job
The Clinical Pharmacist will take responsibility for areas of chronic disease management, undertake clinical medication reviews and proactively manage patients with complex polypharmacy. Working as part of a multidisciplinary team in a patient facing role to support the PCN and Partner Practices.
About us
Cambridge City 4 PCN Limited was established in 2019 to support six GP surgeries in Central Cambridge, as part of the national PCN DES contract. The Network has grown and developed significantly since this time and we now employ a highly skilled multidisciplinary team of thirty, consisting of Pharmacy, Personalised Care and Physiotherapy professionals. The Network is lead by two Clinical Directors who are GP Partners in two of our Partner practices and they are supported by our PCN Manager and our Strategy and Finance Officer. The Network became a Limited Company in 2021 and has six Company Directors, one from each of our Partner practices.
Cambridge City 4 PCN Limited provides a supportive, inclusive and innovative environment for our workforce and strives to develop our employees to reach their full professional potential. As part of the Pharmacy team you will be supported throughout the CPPE pathway, Independent Prescriber qualifications and beyond with great importance placed on taking a patient facing and focussed role.
Each of our Pharmacists is supported by a dedicated Pharmacy Technician - as a team of two they work together within two surgeries and are recognised as valuable members of the practice and PCN teams. We operate a 'hub' model once per week during which time the team come together to work from the same location in order to access face to face peer support, protected learning, mentoring, shadowing and relationship building opportunities.

Person Specification
Qualifications
Essential
Masters degree in Pharmacy (MPharm) or equivalent.
Desirable
Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience. Independent Prescriber (or working towards/intent of gaining IP status).
Experience
Essential
Minimum of two years post qualification experience. An understanding of the nature of General Practice. Excellent interpersonal, influencing and negotiating skills. Excellent written and verbal communication skills. Ability to communicate complex and sensitive information in an understandable form to a variety of audiences. Ability to plan, manage, monitor, advise and review general medicine optimisation issues in core areas of long term conditions. Excellent IT skills. Ability to mentor and motivate other members of the Pharmacy team. Ability to work under pressure and meet deadlines. Self motivated and adaptable.
Desirable
In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare. Experience of working within a GP surgery or PCN environment. Independent Prescriber.
