We are looking for an Advanced Practitioner - Transitional Role to join our team in Lancashire Young Persons service, primarily based in our central office in Preston. This will involve managing a caseload of complex clients with multiple needs.

You will need to be experienced in substance misuse and mental health and have the ability to create rapport quickly with both clients and professionals.

You will take a lead in supporting and upskilling recovery workers, offering coaching and development around working with people with co-occurring needs. This is an exciting opportunity to be part of a team, who offer a flexible and person centred support to those who need it most.

**About the job**

You will lead the way in clinical governance for our local service to ensure the needs of those seeking our support are met in an appropriate way from their assessment right through to any necessary referral.

You will have a reduced caseload of your own to manage, some of which may be complex.

There may be one or more specialist areas where you'll need to share your best practice to fill any gaps in the team's knowledge or skill requirements. Your support will also be needed to train new team members, lead peer supervision or reflective practice and to address any learning requirements which arise from incidents, audits or complaints.

You'll support and lead changes to practices to improve our overall standards and ensure we comply with Government guidelines and standards.

**REQUIRED SKILLS**:
To carry out this role succesfully, you will:

- hold an ONC/NVQ level 3 or equivalent
- have an FDAP accreditation
- have experience of working in the health, social care, criminal justice or other specialist setting
- have knowledge an understanding of brief therapy, motvational interviewing, group working, assessments and care planning and also rehabilitation
- have knowledge and understanding of health & safety, including assessing high risk situations involving clients
- be aware of the issues affecting those who misuse substances
- have experience of care-coordination and caseload management
- have experience of supporting others with the induction and development of new staff
- be able to demonstrate building and developing strong and positive relationships with key stakeholders to ensure continuity of service
- be a great team player and able to build successful working relationships with a range of different people
- have excellent organisational skills, able to manage your time and work effectively under pressure
- have excellent written and verbal communication skills and be comfortable using IT packages and digital media
