Our award-winning Integrated Frailty Team are expanding, and we want you to be part of our next brilliant development Due to continued investment within the team we’re recruiting to multiple new positions ranging from band 5 to band 8a.

This exciting new role is a fully funded training position, you will be supported through a 3 year master’s degree programme to compliment your professional development in Frailty. Annex 21 is utilised for the training role, resulting in progression to band 8a upon completion of the course. You will attend university 1 day per week, have 7.5 hours additional study time per week and then spend the remaining days/hours working in the Integrated Frailty Team.

Our team is integrated, consisting of occupational therapists, physiotherapists, therapy assistants, nursing colleagues and doctors. This results in holistic care for our patients and plenty of support, especially when complex decisions need to be made. As a team we believe in patients receiving the right care, in the right place, at the right time and will support our patients to safely avoid an admission to hospital wherever possible.

There are plans for the service to move to 7 day working and extended hours on the Preston site, and in the future at the Chorley site, therefore you will require a flexible approach to working hours.

Clinically, you will be a key member of the multi-disciplinary Acute Frailty Team, supported by 3 Consultant Geriatrician’s and a Lead Frailty Nurse Practitioner. Demonstrating advanced and expert knowledge and skills in the autonomous therapeutic management of frailty patients within acute rapid assessment areas. You will also act as a source of expertise for the therapy management of frailty, utilising evidence based practice.

Under the research pillar your role will be to continually review the standards of practice and efficacy/efficiency of service delivery to patients and their carers throughout your area of responsibility and recommend changes in line with national service frameworks and clinical guidelines. During your training and once fully qualified you will undertake and facilitate relevant research in the clinical area, in line with hospital policies and to present and publish research outcomes.

For the leadership pillar, you will have several responsibilities. Firstly, providing over-arching daily leadership for the therapy team within Acute Frailty Service areas and direct line management of the 2 Clinical Lead Therapists in the team. Along with the MDT leadership team you will evaluate the day to day running and development of the service, develop policies/procedures /guidelines and promote good governance. Lastly, you will support business manager colleagues with finance and performance related tasks.

**Duties will include but not limited to**:
To practice autonomously with a high level of accountability at an advanced specialist level in the direct delivery of high quality care.

Uses advanced therapeutic clinical skills to effectively assess and manage frail patients presenting to the Trust.

Plan and manage complete episodes of care, working in partnership with others and delegating and referring as appropriate to optimise health outcomes and resource use, as well as providing direct support to patients and clients.

To provide clear leadership and direction to support the Frailty Therapy Team and wider MDT, acting as a role model for the Trust, presenting a positive image to patients and staff.

Advocate and contribute to the development of an organisational culture that supports continuous learning and development, evidence-based practice and succession planning.

Critically engage in research activity, adhering to good research practice guidance, so that evidence based strategies are developed and applied to enhance quality, safety, productivity and value for money.
