Junior Clinical Fellow Frailty

You will be part of the Elderly Care team on an Acute Frailty Assessment Unit (AFAU). You will work with the multidisciplinary team to provide excellent care to our patients.
You will be supervised by the consultant geriatricians within our department.
Be involved directly with the day to day functioning of the Acute Frailty Assessment Unit, and support with the provision of frailty in reach into the Emergency Department.
Participate in multidisciplinary team working (including physiotherapy, occupational therapy, speech and language therapy, mental health liaison and palliative care) to promote patient independence and wellbeing, and proactive discharge planning.
Participate in consultant-led ward rounds and perform ward rounds with supervision.
Undertake the administrative duties involved with the care of your patients.
Perform procedures (venepuncture, cannulation, arterial blood gas sampling, NG tube placement, catheters, mental state examination)
Develop communication skills (including obtaining collateral histories, conducting family meetings and addressing advance care planning needs)
Promote high professional standards of clinical care for patients, both through direct patient care and by the maintenance of continuing professional development.
Maintain continuing education and professional development.
Actively participate in the audit process and the quality improvement activity in the department.
