**Job Opportunity: Lead Practitioner**

Location - North Lanarkshire

As a Lead Practitioner, you will play a crucial role in providing exceptional care and support to individuals who rely on their services.
**Key Responsibilities**:

- Assisting adults by offering tailored practical and emotional assistance to empower them in reaching their unique personal goals.
- Providing support across various aspects of their lives, including engaging in meaningful activities, managing appointments, participating in the community, maintaining physical health, fostering relationships, and enhancing emotional well-being.
- Addressing individuals in crisis and utilizing your expertise to assess their needs through information gathering.
- Collaborating with statutory services to ensure a customized approach that aligns with the individual's requirements.
- Working with them on a short-term basis to navigate and overcome crisis situations, collaboratively developing a SMART plan with attainable outcomes.
- Taking part in events and information sessions to both attend and host, promoting the service and creating awareness.
