Flow Rehabilitation Coordinator

**The Role**:
The post holder will be aligned to the frailty clinical network and will work as an expert clinical practitioner responsible for coordinating the transfer of frail patients to community beds for ongoing rehabilitation, HBCCC and interim care. As part of a multi-disciplinary/ multi-agency team, in line with local processes, will aim to prioritise the right person for the right beds to improve patient outcomes and journey.

The post holder will be an experienced practitioner, who will act within their professional boundaries and provide expert advice and coordination support to multidisciplinary teams (clinical and non-clinical) across acute and community settings within both the health and social care partnerships and other agencies. The practitioner must demonstrate competence within all four pillars of practice and make professional, autonomous decisions for which he/she is accountable relating to the seamless transfer of patients to off-site facilities to help maintain patient flow.

**What we'll need you to bring**:

- Educated to degree level with a willingness to undertake a Masters degree, if not already in procession of this
- Demonstrable experience at a senior level in health care
- The post holder must be a registered practitioner with HCPC or NMC
- Experience in a management role - proven leadership skills and experience are essential
- Use quality improvement methodology to progress service improvements and evaluate the impact on the length of in-patient stay and acute flow
- Development and management of a single pathway for the movement of frail patients to community hospitals to improve patient safety and communication
- To work alongside existing MDT teams, to provide expert advice and recommended solutions regarding the different options for ongoing rehabilitation including community sites and home first approaches

**It would be great if you also have**:

- Experience of the rehabilitation environments and processes
- Experience of planning and delivering rehabilitation and its processes

**Contract type**:
Fixed Term or Secondment for 12 months
