Here, your main responsibility will involve supporting VANL’s Community Solutions programme.

Community Solutions is an exciting and effective partnership programme which supports community and voluntary organisations to improve people’s wellbeing, promote equality and provide support to vulnerable people across North Lanarkshire.

Some of these vulnerable groups include carers, children and young people and those dealing with mental health and wellbeing issues.

The post holder will work to facilitate effective joint planning, deliver social prescribing (helping patients to improve their health, wellbeing and social welfare by connecting them to community services) and improve cancer journey elements of the Community Solutions Improving Lives Initiative.

Your main responsibilities will include:

- Supporting in the development of protocols relating to:

- Delivery of social prescribing for people with health challenges
- Recording and sharing data about individuals supported
- Support the agreement of evaluation and reporting arrangements for Community Solutions work and assist in the delivery of this
- Assist in the design and delivery of induction, training and ongoing support to ten Community Connectors
- Assist with community and voluntary sector capacity-building to improve and develop community support for people living with cancer and other long-term conditions. This will include identifying and seeking additional funding where appropriate
- Assist in the facilitation of a new North Lanarkshire cross-sector and multi-disciplinary Social Prescribing Improvement Group which will agree improvements to social prescribing including approaches, organisational and professional guidance and public information
- Support effective communication and promotion of the Improving Lives Initiative overall with a focus on social prescribing, Improving Cancer Journey and the role of Community Connectors

**Requirements**:
To be successful in the role, you will need:

- At least two or three-years relevant experience in a paid role
- Relevant degree-level qualification
- Commitment to making a positive contribution to residents and communities in North Lanarkshire
- Understanding of health and social care and the role of the community and voluntary sector
- Commitment to and understanding of the mission and values of VANL or a willingness to develop these
- A motivated attitude with a willingness to reflect, learn and improve
- Excellent organisational and administrative skills
- Excellent verbal and written communication skills
- Able to capture, analyse and report on data and information accurately and clearly
- Able to plan and manage work independently
- Able to problem-solve and approach challenges constructively and know when to ask for support and guidance
- Able to work well with internal and external colleagues
- Able to use ICT effectively to fulfil working responsibilities
