The post holder will support the delivery of the ICS Primary Care Strategy and the development of the case for change and will work with local system partners to support implementation. The post is responsible for providing professional expertise to the outcome of the business processes for the Department or Directorate. The post holder will be the lead for delivering Primary Care transformation framework and schemes. This is to improve clinical outcomes and reduce unwarranted variation between practices and Primary Care Networks.

This includes but is not limited to improving patient experience of primary care, delivering high quality services, Improving health outcomes via collaborative working, reducing variation and commissioning universal services and ensuring fit for purpose estate and digital alternatives. The post holder will act as the lead for the delivery of national plans in relation to dental, pharmacy and optometrists, in particular the national dental reforms. The post holder will define current processes, facilitate discussions and advise colleagues as to how best practice might be adopted in the definition of future processes, document those processes as they are agreed and oversee their delivery. The post holder will establish and agree new models of care plans with Primary Care Networks and provider organisations, agree outcomes and a process for evaluation and feedback.

The post holder will line manage the Senior Primary Care Managers The post holder will contribute to processes and implement those processes and outcomes of the management of primary medical care contracts and services, as well as the delegated commissioning functions. The post holder will deputise for the Head of Primary Care Contracting and Transformation as required This job description and person specification are only an outline of the tasks, responsibilities and outcomes required of the role.
