Job Title: Primary Care Nurse

Job Summary: We are seeking a skilled and compassionate Primary Care Nurse to join our team at NHS. The successful candidate will work closely with a multi-disciplinary team to deliver high-quality patient care and support.

Key Responsibilities:

  • Deliver nurse-led clinics to patient groups
  • Work collaboratively with a multi-disciplinary team to achieve excellent patient outcomes
  • Develop and implement care plans to meet individual patient needs
  • Provide education and support to patients and their families
  • Contribute to the development of the team through sharing best practices and ideas

Requirements:

  • Registered Nurse Adult qualification
  • Current professional qualification (NMC)
  • Evidence of Continued Professional Development
  • Understanding of clinical governance, health and safety, QOF, EBP, and national standards
