We are looking for 3 Advanced Clinical Practitioners to join our well-established PCN which is comprised of 9 GP practices. Our PCN practices cover much of the Central and South Gateshead population, providing care to over 63,000 patients with diverse health care needs. Each practice runs independently, with an individual practice team but all practices work together on shared ethos of best possible patient care and joint goals for the PCN.

You would be joining an already well established team of Additional Roles staff including Pharmacists, Musculoskeletal First Contact Practitioners, Mental Health Practitioner, Physician Associate, Counsellors and Care Coordinators. Each practitioner would work across the PCN and may be mainly allocated to 3 practices, however there may be some central hub working and also the need for cross cover during holiday times.The job will be patient facing, but may include telephone triage and consultations to allow greater patient access to services. Applicants must have the necessaryAdvanced Clinical Practitionerqualificationand must be an independent prescriber. Job Type: Full-Time 37.5 hours/ week patient facing.
