We are looking for a dynamic end of life care coordinator with a passion for improving the care people receive at end of life.

The service will work closely with existing providers across health and social care enhancing the care offer.

**The main focus of the role will be to**:

- Work as part of a system to support the delivery of safe and effective case management of patients with EOLC needs across a variety of care settings within the community.
- Co-ordinate a virtual caseload of EOL patients, providing support where necessary to ensure additional targeted resources are provided e.g. respite/over night support.
- Sitting alongside the Community Nursing Teams the EOLC co-ordinator will work in partnership with the wider team to offer continuing support and advice to family and carers and provide EOLC nursing advice to colleagues in both primary and secondary care settings, particularly when patients are transferred between the two areas.
- Where necessary work in collaboration to support the community staff and teams in the review and risk assessment of all EOL care packages to ensure that care needs are being met within the funding parameters and within the appropriate timeframes.
- Ensure that effective and timely communication regarding the review process and the outcome of the review occurs with all professionals involved and with the patient and their families.
- Participate in the promotion of patient-centred care, establishing and maintaining a supportive relationship with the patient and their family.

Probationary Period

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436 per annum pro rata).

**The Post holder will have**:

- Registered Nurse (RGN/RN)
- Palliative care qualification at degree level or equivalent
- Recognised teaching qualification or demonstrable experience
- Advanced Communication skills and managing difficult conversations
- 4 years’ Post-Registration experience, to include caring for patients with palliative care needs
