**About the Role**:
Southend Medical Practice is looking for a motivated individual who has a passion for tackling health inequalities and, in particular, an interest in healthcare for the homeless population.

Southend Integrated Healthcare for the Homeless (SIHH) provides a service specific to homeless individuals within Southend-on-Sea. The role will include a range of activities addressing the individual health and wellbeing, the development of pathways into specialist services, training of staff who are involved in caring for the homeless and improve the overall health inequalities via:

- Health promotion;
- General health screening;
- Engagement in primary care services and facilitating access to organised health care;
- Managing integration of the service with secondary care services, mental health services and local drug and alcohol services;
- Working closely with local voluntary providers involved in looking after the local homeless population.

This role will be imperative to improving integrated pathways, ensuring that they are efficient and effective, and preventing fragmented services, in order to improve health outcomes.

The collective aim of the service is to help those with a range of health needs, including drug and alcohol dependence, to access the appropriate treatment, and to provide clinical assessment and treatment in the community, signposting clients to GP services, secondary care and drug & alcohol services. There will also be the opportunity to do regular clinical sessions within the lead GPs practice in Southend.

Main responsibilities:

- To provide a specialist primary care service and outreach clinics for local soup kitchen, homeless individuals and local homeless charities in Southend.
- To work as a member of the multidisciplinary team in the management and treatment provision. General health assessment, wound assessment and management, lifestyle assessment and physical health assessment.
- To promote health and wellbeing and improve health inequalities by completing a range of health checks including a review of systems to include, neurological, cardiovascular, respiratory, gastrointestinal, hepatic, renal, lymphatic, reproductive health and musculoskeletal.
- To be able to assess general health and provide early intervention treatment, sign posting and referral to organised health services.
- To develop and maintain pathways into specialist services to ensure the health and well-being of residents is met, including back into primary care services, secondary care, mental health, drugs and alcohol and voluntary sector.
- To promote engagement in primary care services and facilitate access to organised health care.
- To sign post service users to the appropriate health care settings to meet their needs.
- To be the single point of contact for primary care services, local Hospitals and other organised health care services.
- To provide regular drop in clinical sessions at The Bradbury Centre (HARP) and the 2 local soup kitchens.
- To attend MDT meetings with the Homeless Mental Health services.
- To develop and provide a training package for those that work within the hostels on providing brief interventions and dealing with out of hours support.
- To develop and provide an educational training package for GP practices on the role of the service and managing homeless patients registered at a practice.
- To attend mandatory yearly updates on CPR and anaphylaxis training.
- To actively promote female health screening.

We are seeking an experienced nurse or paramedic, with current professional registration, who will work closely with a health care assistant (to be recruited) and will report back to a GP clinical lead. No previous experience in working with this population is needed.
