A fantastic opportunity has arisen for an experienced, organised and efficient Care Coordinator to join our team at Avacare.

As a Care Coordinator you will be working Monday to Friday 9am - 5pm. You may be required to pick up any additional duties of care in the event of an emergency. Due to the nature of the role, you could be travelling to service user's homes.

Duties may include, but may not be limited to: Allocating carers to their assigned service users Completing rotas and ensuring all visits are covered with sufficient well planed travel time and routes. Maintaining client and care worker files. Processing new client referrals. Maintaining excellent communication with your team and service users. Supporting and developing care staff. Stepping into the role as a care worker when needed. Managing the on call service on a rostered basis.

**Job Type**: Permanent

**Salary**: £22,000.00-£25,000.00 per year
