**Duties include (but not limited to)**:

- Allocating the right carer to the right client at the right times
- Provide co-ordination and navigation for clients and their carers across healthcare services.
- Participate in training, supervision, and recruitment of the workforce.
- Maintain excellent communication between the workforce and management.
- Liaising with a variety of healthcare professionals.
- Supporting Health Care Assistants to do their job to the best of their ability.
- Ensure personal care and medication administration is met.
- Contribute to staff development and identify any training needs of Health Care Assistants.
- Taking part in on call rota.

**Experience**:

- Providing care: 2 years (required)
- Manual handling: 2 years (preferred)
- Medication administration: 2 years (preferred)
- Care coordinator: 2 years (required)

Pay: £24,000.00-£30,000.00 per year
