The purpose of this role is to support care providers, discharge teams, patients and their families by carrying out assessments on behalf of care providers. It is aimed that this will free up resources in providers, facilitate discharges from hospital more quickly and smoothly and reduce post-admission problems and readmissions to hospital. We are looking for an additional person to work alongside our existing NHS Band 5 Trusted Assessor.

**Specific Tasks**

1. Carry out assessments of patients across Oxfordshire hospital sites and in the community, including re-started packages of care. These may be patients who were living at home prior to admission and whose needs are being assessed to consider whether they can be discharged home again.

2. Act as a point of liaison for discharge teams, patients and their families, and providers by ensuring information is shared with appropriate parties in relation to potential discharges into care settings, and work to facilitate the flow of information between the hospital, Oxfordshire care providers, Adult Social Care and the Discharge Liaison Team about the person’s needs and the Home Care Services’ ability to meet those needs.

3. Provide information and support to patients and to their families during the discharge process.

4. Support improvements in discharge arrangements to people’s home environment in Oxfordshire, improving patient experience, clinical safety and patient flow including ensuring support requirements are in place to enable safe hospital discharge and Home Care Services admission.

5. Along with participating homes and commissioners, develop documentation and agreements setting out how the scheme works to ensure that participating providers can easily explain it to the Care Quality Commission and meet their own governance and insurance obligations

6. Complete accurate and timely collection, analysis and reporting of data relating to discharges from hospital to care providers to enable the pilot project to be properly evaluated.

7. Identify bottlenecks in processes and implement solutions to aid in reducing delayed discharges of care, adhering to the principles of continuous improvement.

8. Report on issues raised by care providers about quality of discharge, working closely with relevant NHS partners, such as Patient Flow and Discharge Teams, to improve future discharges.

9. The post holder will be required to work flexibly and there may be a requirement for some weekend working.

10. The post holder must have an ability to travel to Oxford and occasionally across Oxfordshire as required.

11. A satisfactory DBS disclosure at the enhanced level is required.

12. The above list of duties is indicative only and not exhaustive. The job holder will be expected to perform all such additional duties as are reasonably commensurate with the role.
